Baptist Memorial Health Care is proud to introduce Hospital at Home, a new way to deliver high-quality, hospital-level care to qualified patients in their homes that improves outcomes and enhances the patient experience.

Hospital at Home

Hospital at Home is a system of care based on a model developed by researchers at the Johns Hopkins Schools of Medicine and Public Health and tested at medical centers across the country. This innovative program provides hospital-level care in a patient's home as an alternative to inpatient hospital care. Baptist's physicians, nurses and other providers deliver the same quality of care you’d receive at a Baptist Memorial Hospital, only in the comfort of your own home.

Safe, Effective Care

Research has shown that well-monitored, at-home treatment can be safe and effective for some patients. The care you receive will closely resemble the care you would get as a patient in the hospital. A nurse or other health care professional will visit you in person twice a day, and you will meet once a day with a physician, nurse practitioner or a physician’s assistant in person or virtually through Baptist OneCare® MyChart.

Hospital at Home candidates will undergo a home assessment to ensure we have the space, Internet connectivity (cell phone service or broadband Wi-Fi) and physical accessibility to deliver home care properly.

And, there is no additional cost for Hospital at Home care. Patients pay the same amount as they would if they were admitted to the hospital.

Available to Medicare Patients

Currently, Hospital at Home is available only to Medicare patients at Mississippi Baptist Medical Center (in Jackson, Mississippi) who have one of the following diagnoses:

  • Heart failure
  • Chronic obstructive pulmonary disease (COPD)
  • Urinary tract infection
  • Hyperglycemia
  • Cellulitis
  • Pneumonia
  • Dehydration
  • Deep vein thrombosis

Patients must qualify for an in-hospital admission to be eligible for Hospital at Home. If you qualify for the service, we will transport you home and give you all the equipment (i.e., heart monitor, blood pressure monitor, IV set up, etc.) necessary to monitor and measure your progress. If necessary, we will send you a hospital bed and set it up for you. We will also deliver meals to your home, prepared by the hospital’s food service team.
